The below five situations call for Sajdah’e Sahv…
For talking forgetfully in Namaz.
نماز کی حالت میں بھول کر کلام (بات) کرنا۔
Reciting Salaam at the wrong place.
نماز میں سلام کو آخری رکعت کے علاوہ انجام دینا۔
Forgetting Tashahud.
تشہد کا بھولنا۔
When there is a doubt in a 4 Rak’at salaat, after 2nd Sajdah, as to whether the number of rak’ats performed is 4 or 5, 4 or 6.
چار رکعت نماز میں دوسرے سجدے کے بعد یہ شک ہو کہ یہ چوتھی یا پانچویں، چوتھی یا چھٹی ہے۔
When after salaat, one realises that he has either omitted or added something by mistake, but that omission or addition does not render the prayer void.

Method of Sajdah-e-Sahv…
Immediately after the Salaam of Namaz, one should make a niyyat of performing Sajdah’e Sahv and do 2 Sajdahs; in each Sajdah he should recite as follows…
سلام نماز کے فورًا بعد چاہیے کہ سجدۂ سہو کی نیت کرے اور ۲ سجدہ کرے جس کے ہر سجدہ میں اس طرح سے کہے۔۔۔
بِسْمِ اللہِ وَبِاللہِ اَلسَّلَامُ عَلَیْكَ اَیُّہَا النَّبِیُّ وَرَحْمَۃُ اللہِ وَبَرَكَاتُہٗ۔
Bismi allaahi wabillaahi as-salaamu 'alayka ayyuhaa an-nabiyyu warahmatu allaahi wabarakaatuh.
In the name of Allah and by Allah. O Prophet! Allah’s peace, blessings and grace be upon you!
اللہ کے نام سے اور اللہ سے۔ اے پیغمبر! اللہ کا درود، رحمت اور برکت ہو آپ پر۔
After performing the 2nd Sajdah one should sit up again and recite Tashahud as follow…
دوسرے سجدے کے بعد بیٹھے رہے اور اس طرح سے تشہد پڑھے۔۔۔
اَشْهَدُ اَن لَّا اِلٰهَ اِلَّا اللهُ وَحْدَهُ لَا شَرِیْكَ لَهُ، وَاَشْهَدُ اَنَّ مُحَمَّدًا عَبْدُهُ وَرَسُوْلُهُ، اَللّٰهُمَّ صَلِّ عَلٰى مُحَمَّدٍ وَّآلِ مُحَمَّدٍ۔
Ashhadu an laa ilaaha illaa allaahu wahdahu laa shareeka lahu, wa-ashhadu anna muhammadan 'abduhu warasooluhu, allaahumma salli 'alaa muhammadin wa-aali muhammad.
I testify that there is no god but Allah, He alone, for whom there is no partner. And I testify that Muhammad is His servant and His messenger. O Allah! Bless Muhammad (s.a.w.a) and the progeny of Muhammad (a.s).
میں گواہی دیتا ہوں کہ سوائے اس خدا کے جو یکتا ہے اور جس کا کوئی شریک نہیں کوئی اور خدا پرستش کے لائق نہیں ہے اور میں گواہی دیتا ہوں کہ محمدؐ خدا کے بندے اور اس کے رسول ہیں۔ اے خدا رحمت بھیج محمدؐ اور آل محمدؑ پر۔
After Tashahud recite Salaam as follow…
تشہد کے بعد اس طرح سے سلام پڑھے۔۔۔
اَلسَّلَامُ عَلَیْكُمْ وَرَحْمَةُ اللهِ وَبَرَکَاتُهُ۔
As-salaamu 'alaykum warahmatu allaahi wabarakaatuh.
Peace be upon you, and the mercy of Allah, and His blessings.
آپ پر سلامتی ہو، اور اللہ کی رحمت اور برکتیں ہوں۔
